The SED Support Coordinator will be responsible for assessing the needs and strengths of children and youth with serious emotional disturbance (SED) and their families; providing support coordination, advocacy, service linkage, and ongoing care coordination to ensure access to appropriate behavioral health, developmental, educational, medical, and community‑based services. The SED Support Coordinator will provide face‑to‑face support, outreach, and coordination with children, youth, parents/caregivers, schools, healthcare providers, behavioral health providers, social services, and other community partners to promote stability, safety, and improved functioning across home, school, and community settings. Work is performed under regular supervision.
General knowledge of community and agency resources and programs of regional and state sources for the community service population; general knowledge of interviewing and supportive counseling techniques; general knowledge of social casework techniques; general knowledge of human development and behavior; general knowledge of the theories, principles and techniques of individual, family and group therapy; knowledge of principles and practices related to case management, care coordination, and service planning; strong organizational and time‑management skills; ability to prioritize and manage multiple responsibilities effectively; ability to work independently and as part of a team; ability to adapt to changing client and program needs; ability to work a flexible schedule, including occasional evenings or other non‑traditional hours, as required by client and program needs; ability to solve problems within scope of responsibility; ability to analyze facts and exercise sound judgment in arriving at conclusions; ability to communicate complex ideas effectively, orally and in writing; ability to prepare clear and concise reports; ability to establish and maintain effective working relationships with clients, associates and the general public; general knowledge of child development and techniques for working with special needs and/or emotionally disturbed children and adolescents; knowledge of MS Office, to include Word and Excel.

Bachelor’s degree in Human Services or equivalent. Preferred experience working in the behavioral health or human services field and/or working with children, youth, and adolescents. Experience with case management, care coordination, or community-based services is preferred.
This work requires little or no physical effort or exertion. Working conditions are good; environment may be slightly dirty or involve occasional exposure to some disagreeable elements; accidents are improbable other than minor injuries; health hazards negligible; some mental effort and stress may be involved resulting in inconvenience and frustration. The worker is subject to inside and outside environmental conditions.
Valid Virginia driver’s license; attends training and maintains certifications as required by the Agency and regulatory guidelines, such as CPR, First Aid, High Fidelity Wraparound Training for Intensive Care Coordination, and DBHDS Case Management Modules.
