Job summary

Easter Seals Louisiana is seeking a dedicated Support Coordinator to assess needs, develop Comprehensive Plans of Care, and coordinate services for participants in the NOW/EPSDT/Children’s Choice programs. You will work closely with families, physicians, and service providers, maintain accurate records, and ensure timely plan reviews while meeting OAAS/ESL timelines.

Qualifications

• Bachelor’s degree in social services or a related field.
• Proven ability to multitask and meet deadlines.
• Excellent written and verbal communication skills.
• Proficiency in MS Word and MS Excel.

Responsibilities

• Assess and address Comprehensive Plan of Care (CPOC) needs expressed by participant/family and professionals.
• Document and follow up to ensure services are in place and appropriate.
• Maintain thorough, up-to-date progress reports.
• Travel to participant locations as required.
• Meet with supervisor for supervision and participate in staff meetings.
• Ensure compliance with OAAS/ESL timelines and program requirements.
