Job summary

JD Home Healthcare in Saint Cloud, MN is seeking a Direct Support Professional to provide comprehensive personal care, guidance, and support to clients, promoting independence and implementing individualized plans.

You will assist with activities of daily living, accompany clients in recreation, track data, attend team meetings, and help implement Behavior Support Plans and ISPs according to state requirements.

Qualifications

• Must be 18 years or older.
• High School Diploma, GED, or equivalent.
• Pass background check with MN DHS.
• General computer skills and reliable transportation in Minnesota.
• Current driver’s license and proof of insurance.
• Clear driving record (No DUI/DWI in past 5 years).
• Fluency in reading, speaking, and writing in English.
• Dynamic communication skills.
• Working cell phone with internet access.

Responsibilities

• Assist clients in behavioral and personal care services.
• Assist with activities of daily living per care plans.
• Accompany clients on recreation and leisure activities.
• Implement clients' plans as written.
• Increase clients' independence and community involvement.
• Understand and implement Behavior Support Plans and ISPs.
• Track and chart data daily per company expectations.
• Attend, assist, and engage in team meetings as needed.
