The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking several full‑time Support Administrators (SA) to facilitate the development of person‑centered plans that promote self‑determination. They will develop, authorize and revise a personal budget based on needs and outcomes identified in the plan, assist individuals and their families to explore and obtain services and supports, and implement an ongoing system of review.

  • Act as the primary point of coordination for the person‑centered planning team and utilize an assess/plan/review/revise process to help teams plan for each individual assigned.
  • Assist individuals and teams to identify issues, areas of potential risk, and important outcomes, and coordinate thorough assessments to determine the need for services and supports, revising as needs change and annually.
  • Facilitate the development of a person‑centered plan, including the determination and direction of needed supports, revising the plan as needed based on individual or team concerns, progress toward outcomes, or other impacting factors.
  • Identify funding sources for supports defined in the Individualized Service Plan (ISP) using a hierarchy of applicable funding options, and plan with teams to access cost‑efficient services.
  • Establish and approve budgets for services based on assessed needs and preferred methods of meeting those needs; manage fiscal processes related to authorization and response to revisions.
  • Maintain contact with service providers and natural supports to ensure effective coordination.
  • Promote community presence and participation by providing referrals and linkage to community resources.
  • Implement continuous monitoring of plan review to ensure supports are delivered, outcomes achieved, and individuals are satisfied, taking action when concerns arise.
  • Assist individuals in selecting providers as needed.
  • Assist with enrollment onto Medicaid waivers, completing all related paperwork accurately to maintain funding for services.
  • Determine an individual’s eligibility for County Board of DD services.
