Job Summary

The Supplemental Care Manager supports the Value Based Care Management program by providing high-quality telephonic Case or Care Management with CareFirst members. They ensure the ongoing needs of enrolled members, caregivers, and providers are met, maintaining strong relationships while the primary Care Manager is unavailable. The role involves advocacy, guiding using motivational interviewing, and outreach to prospective members.

Essential Job Functions

• Advocate, guide, and intervene on behalf of members to ensure successful implementation of Care Plans.
• Conduct follow-up calls with members as indicated.
• Assist members with coordination of tests, images, and specialist consults.
• Identify benefits and community resources, evaluate Social determinants of Health, and facilitate referrals.
• Document care plans and member progress accurately.
• Maintain communication between CareFirst, primary care practices, and Care Coordination Team.
• Follow program guidelines and meet productivity and quality metrics.
• Complete training and participate in team activities.
• Engage new members via automated systems to encourage program participation.
