Job Summary

The Supplemental Care Manager supports the Value Based Care Management program by providing high-quality telephonic Case or Care Management with CareFirst members. This role ensures that the ongoing needs of enrolled members, caregivers, and providers are met and maintains strong working relationships while the primary Care Manager is unavailable (e.g., leave of absence, PTO). The manager advocates and guides using motivational interviewing techniques, intervenes on behalf of members, and performs outreach to prospective members to engage them into the program.

Essential Job Functions
  • Advocate, guide, and intervene on behalf of members to implement their Care Plans successfully.
  • Conduct follow-up calls with members as indicated.
  • Assist members in coordinating tests, images, and specialist consults.
  • Identify relevant benefits and community resources, evaluate Social determinants of Health, and facilitate referrals.
  • Conduct member education and medication reconciliation using appropriate modalities.
  • Maintain quality documentation of care plans, progress, and barriers.
  • Ensure seamless communication between CareFirst, primary care practices, and the Care Coordination Team.
  • Follow program guidelines and meet productivity and quality metrics.
  • Complete required training and participate in team activities.
  • Outreach new members via automated systems to engage them in the program.
Skills and Attributes
  • Strong motivational interviewing and case management skills.
  • Self-directed, organized, and capable of multitasking.
  • Excellent communication and customer service skills.
  • Resilient and adaptable in a fast-paced environment.
  • Experience with medical records and care plan documentation.
Qualifications
  • Current multi-state Registered Nurse licensure; BSN preferred.
  • Training in motivational interviewing preferred.
  • 3-5 years of clinical experience with telephonic Case Management preferred.
  • Proficiency with electronic medical records and Microsoft Office suite.
  • Home office with HIPAA compliance and reliable internet required.
