Hackensack Meridian Health is seeking a Supervisor, Clinical Therapists to oversee a team of Clinical Therapists, manage caseloads, and plan staffing to meet patient care needs in an acute care setting.

The role integrates care coordination, discharge planning, and performance improvement, ensuring evidence-based practice and regulatory compliance. Hours are Monday–Friday, 7:00 a.m.–3:30 p.m.

Qualifications

  • Master's degree in Social Work (MSW) required.
  • Minimum of 5 years of post-advanced-license experience, including 3 years in an acute care setting.
  • Care coordination, case management, or discharge planning experience.
  • Proficiency with policy review and program planning.
  • Ability to work as part of a team with strong collaboration.
  • Excellent follow-through, attention to detail, written and verbal communication.

Responsibilities

  • Promote efficient utilization of resources and ensure care meets standards and regulatory requirements.

The Supervisor, Clinical Therapists is accountable for overseeing the department's designated team members' caseloads and planning effectively in order to meet staffing needs, manage length of stay, promote efficient utilization of resources, and ensure that care meets evidence-based practice standards and regulatory/payor requirements.

The role integrates care coordination, discharge planning functions, and performance improvement activities for all Clinical Therapists.

Hours are: Monday - Friday 7:00 a.m. - 3:30 p.m.

Education, Knowledge, Skills and Abilities Required:
  • Master's degree in Social Work (MSW).
  • Minimum of 5 years of post-advanced-license experience, to include 3 years of full-time experience in an acute care setting.
  • Care coordination, case management, or discharge planning experience.
  • Proficiency with policy review and program planning.
  • Ability to work as part of a team, while building collaborative relationships.
  • Must possess excellent follow-through and attention to detail.
  • Must possess excellent written and verbal communication and presentation skills.
  • Familiar with hospital resources, community resources, and/or resource/utilization management.
  • Effective decision-making /problem-solving skills, demonstration of creativity in problem-solving, and influential leadership skills.
  • Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.
Licenses and Certifications Required:
  • NJ Licensed Clinical Social Worker.
Licenses and Certifications Preferred:
  • Basic Cardiac Life Support Certificate.
  • Case Manager Certified.
