The role you’ll contribute:

The Care Management Supervisor, under the general leadership of the Care Management Manager or Facility Director, assists in monitoring and performing daily operations and supervising team members in the Care Management department. This role serves as the primary contact for Care Managers, Social Workers, and patients to address barriers or concerns on inpatient units. The supervisor supports positive patient outcomes, employee evaluations, and customer experience.

The value you’ll bring to the team:
  • Supervise daily discharge planning and social services in inpatient and emergency settings, including direct supervision of Care Management staff.
  • Act as the main point of contact for inpatient unit barriers or concerns related to Care Management.
  • Manage your own assignments and patient/department outcomes of those under your supervision.
  • Assist in implementing corporate Care Management strategies.
  • Ensure active staff participation in daily multidisciplinary rounds.
  • Serve as a resource regarding medical necessity, patient status, and discharge criteria.
Qualifications:
  • Associate Degree in Nursing (RN) or Bachelor of Social Work (BSW).
  • At least 2 years of experience in direct Care Management.
  • State licensure as an RN or Social Worker, including compact state licensure if applicable.
