Job summary

Qualifications

• Ability to assess mental health and substance use needs and develop treatment plans.
• Ability to work independently and as part of a multidisciplinary team.
• Maintains confidentiality per HIPAA and 42 CFR Part II.

Responsibilities

• Provide substance use assessments and make referrals as needed.
• Develop and discuss treatment plans with the healthcare team.
• Deliver brief individual, family, or group counseling to reach therapeutic goals.
• Maintain timely documentation of services, including assessments and referrals.
• Represent the practice in the community and participate in outreach and education.
