Job summary

County of Chester is seeking a Program Specialist I to perform SUD triage, screening/assessment, and case management for justice-involved individuals. You will coordinate with court and probation staff to ensure treatment engagement and community reintegration, while maintaining HIPAA-compliant records and reporting.

Position emphasizes collaboration with criminal justice partners, delivering referrals to housing, healthcare, and support services, and advocating for clients throughout the care

Qualifications

• Bachelor’s degree from an accredited college or university, in Psychology, Counseling, Social Work, Human Services, or related field.
• Four years of professional experience involving substance use disorder services and/or program development, program monitoring, or the delivery of program services.
• Related education may substitute for required experience and related experience may substitute for required education.

Responsibilities

• Completes SUD screening and/or assessment for justice-involved individuals at the Criminal Justice Center.
• Collaborates with supervising officers and criminal justice partners to support court-ordered conditions and promote community integration.
• Ensures timely communication of clinical recommendations and treatment level of care determinations to court and stakeholders.
• Ensures appropriate and timely referrals to substance use treatment providers and community resources.
• Provides case management to individuals re-entering the community to support engagement and continuity of care.
• Maintains documentation and communicates with treatment providers, criminal justice agencies, and others to coordinate services.
