Chester County, PA is seeking a Program Specialist I to perform SUD triage, screening, and assessment for justice-involved individuals. The role focuses on court-related cases, re-entry support, and linkage to treatment and recovery services within a collaborative county system.

Qualifications

• System of care practice model familiarity across child and adult serving agencies and community partners.
• Demonstrated ability to coordinate with multiple justice and treatment partners.
• Ability to maintain compliance with department, state, and federal regulations.

Responsibilities

• Completes Substance Use Disorder (SUD) Screening and/or Assessment for justice-involved individuals at the Criminal Justice Center.
• Collaborates with supervising officers and criminal justice partners to support court-ordered conditions and treatment needs.
• Ensures timely communication of clinical recommendations and levels of care to court and stakeholders.
• Provides case management to individuals re-entering the community from incarceration to support engagement and continuity of care.
• Assists clients with housing, transportation, medical care, mental health services, benefits and other community resources.
