Job summary

GateHouseTreatment is seeking a Case Manager to join our multidisciplinary treatment team. Reporting to the Clinical Supervisor, you will work directly with clients throughout their treatment, helping them identify needs, access resources, build life skills, and prepare for recovery after discharge.

This hands-on, client-focused role requires organization, compassion, and collaboration within a behavioral health team.

Qualifications

• HS diploma or equivalent.
• Strong communication and interpersonal skills.
• Ability to work with individuals in substance use treatment.
• Basic computer skills and ability to document accurately in EHR.
• Ability to collaborate with a multidisciplinary team.
• Preferred: Bachelor’s in human services and CRSW credential.
• Preferred: experience in substance use treatment or case management.

Responsibilities

• Provide case management based on each client’s treatment plan.
• Connect clients with community resources and services.
• Coordinate care with clinical, medical, housing, and operations staff.
• Assist with intake, orientation, and initial service needs.
• Develop realistic aftercare and discharge plans.
• Engage family or supports in discharge planning.
• Maintain accurate documentation in the EHR.
• Participate in treatment team meetings.
• Maintain client confidentiality and professional boundaries.
