Job summary

GateHouseTreatment is seeking a dedicated Case Manager to join our multidisciplinary treatment team in Nashua, NH. You will work directly with clients to identify needs, access resources, build practical life skills, and prepare for continued recovery after discharge.

This hands-on, client-focused position requires organization, compassion, and teamwork within a collaborative behavioral health environment. You’ll engage families and coordinate care across services.

Qualifications

• High school diploma or equivalent.
• Strong communication and interpersonal skills.
• Ability to work professionally and compassionately with individuals receiving substance use treatment.
• Basic computer skills and ability to complete accurate electronic documentation.
• Ability to collaborate effectively with a multidisciplinary team.

Responsibilities

• Provide case management services based on each client’s treatment plan and needs.
• Connect clients with community resources, medical providers, housing assistance, employment services, recovery supports, and other needed services.
• Work closely with clinical, medical, housing, and operations staff to coordinate care.
• Help clients develop realistic aftercare and discharge plans beginning early in treatment.
• Assist clients with identifying recovery goals, addressing barriers, and building skills that support long-term stability.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Assist with client intake, orientation, and coordination of initial service needs.
• Engage family members or other supports in the treatment and discharge-planning process when appropriate.
• Facilitate assigned psychoeducational, life-skills, or recovery-focused groups.
• Participate in treatment team meetings and provide updates on client needs, progress, and discharge planning.
