Group Health Cooperative of South Central Wisconsin (GHC-SCW) seeks a Substance Use Disorder Advanced Practice Provider (SUD APP) for a .5 FTE/20 hours weekly position. The APP collaborates with a multi-disciplinary team to deliver evidence-based outpatient SUD care and coordinates services across behavioral health and primary care settings.

Qualifications

• APNP/PA credentials required per Wisconsin scope

Responsibilities

• Provide outpatient evaluation and treatment for substance use disorders
• Prescribe and manage medications for SUD and related conditions
• Collaborate with physicians, nurses, and counselors to coordinate care
• Obtain and maintain patient records in EPIC and ensure HIPAA compliance
• Engage in patient education and consultation services across behavioral health
• Be available for some evening hours and after-hours on-call
