• Strong communication and documentation skills to support treatment planning.

Responsibilities

• Meet with clients to discuss goals and preferences for independence.
• Contribute to person-centered treatment planning and progress notes.
• Provide evidence-based counseling with Motivational Interviewing and CBT.
• Collaborate with multidisciplinary team for treatment planning and reviews.
• Support clients to achieve recovery goals and coordinate care.
