Job summary

Crossroads Treatment Centers in Tennessee seeks a Substance Abuse Counselor to provide individual counseling to highly complex patients, conduct clinical assessments, and design/update treatment plans including care coordination and discharge planning.

You will manage a 40-hour week with a Saturday rotation; collaborate with nurses and providers to deliver holistic care, and facilitate urine drug screening to ensure compliance and progress in recovery.

Qualifications

• General experience working with underserved populations in a clinical setting.
• Lived addiction experience is a plus.
• Candidates hold one or more of LMSW, LCSW, LPC, or LADAC licensures/certifications.

Responsibilities

• Provides individual counseling to highly complex patients.
• Conducts clinical assessments to appropriately assess patient’s treatment needs.
• Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning.
• Manages assigned caseload and compliance with clinical treatment requirements.
• Collaborates with nurses and providers to provide holistic treatment services.
• Facilitates urine drug screening for patients.
