Job summary

Center for Community Alternatives, Inc. in New York City seeks a dedicated counselor to provide evidence-based counseling services, identify and assess clients, and coordinate care.

The role includes referrals to community-based resources and facilitating therapeutic group and individual sessions. You will maintain OASAS-compliant case records, complete discharge documentation, and collaborate with Crossroads and external providers.

Qualifications

• Intake interviews and biopsychosocials conducted.
• Develop treatment plans and reviews, coordinate care.
• Lead individual counseling and group sessions with clients.
• Maintain detailed, compliant client records in the EHR.
• Facilitate treatment groups and conduct drug testing as needed.
• Prepare monthly court updates and engage in team reviews.

Responsibilities

• Conduct intake interviews, biopsychosocials, mental health screenings.
• Formulate treatment plans and coordinate care with Crossroads and external providers.
• Provide weekly individual counseling and make referrals to community resources.
• Maintain client records per policy; ensure data entry into the agency’s database.
• Facilitate assigned treatment groups; perform drug testing as needed.
• Prepare monthly updates for court or supervising agencies; participate in team reviews.
