Job summary

Center for Community Alternatives, Inc. in New York City seeks a dedicated counselor to provide evidence-based counseling services, identify and assess clients, and coordinate care.

The role includes referrals to community-based resources and facilitating therapeutic group and individual sessions. You will maintain OASAS-compliant case records, complete discharge documentation, and collaborate with Crossroads and external providers.

Qualifications

Job Summary

Provides counseling services including identifying, screening and assessing clients to determine the need for substance use treatment. Provides referrals to community-based organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual sessions. Responsible for completing and maintaining detailed OASAS-compliant case records.

Duties and Responsibilities

• Conduct intake interviews, biopsychosocials, mental health screenings
• Formulate treatment plans and treatment plan reviews, act as participant’s case manager to coordinate care within Crossroads and will external providers
• Perform individual counseling duties which include holding weekly individual sessions with clients, providing referrals to community-based resources (healthcare, social services, and administrative e.g. help obtaining insurance), and developing client schedules
• Initiate and maintain client records in accordance with policies and procedures; ensure accurate and timely data entry of all client information into the agency’s database; utilize electronic health record system
• Facilitate assigned treatment groups
• Perform drug testing as needed
• Prepare monthly updates for court or other supervising agencies
• Participate in treatment team case reviews.
• Consult with other professionals in support of client’s treatment
• Complete discharge documentation as needed
