Job summary

Center for Community Alternatives (CCA) hires a counselor to provide counseling services identifying, screening and assessing clients for substance use treatment, and to coordinate care as needed. Responsibilities include facilitating group and individual sessions and maintaining OASAS-compliant case records.

The role requires collaboration with Crossroads and external providers, developing treatment plans, and ensuring timely data entry.

Responsibilities

  • Conduct intake interviews, biopsychosocials, mental health screenings
  • Formulate treatment plans and treatment plan reviews, act as participant’s case manager to coordinate care within Crossroads and with external providers
  • Perform individual counseling duties which include holding weekly individual sessions with clients, providing referrals to community-based resources (healthcare, social services, and administrative e.g. help obtaining insurance), and developing client schedules
  • Initiate and maintain client records in accordance with policies and procedures; ensure data entry into the agency’s database; utilize electronic health record system
  • Facilitate assigned treatment groups
  • Perform drug testing as needed
  • Prepare monthly updates for court or other supervising agencies
  • Participate in treatment team case reviews
  • Consult with other professionals in support of client’s treatment
  • Complete discharge documentation as needed
