Qualifications

• Bachelor's degree in Sociology, Psychology, Social Work, or Human Services required.
• Two (2) years of related experience required.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct extensive outreach and assessments to determine individual and family needs.
• Maintain accurate, neat, timely documentation of services provided on client files.
• Provide Motivational Interviewing and support for discharge planning during and after interventions.
• Engage clients and their families to improve retention and outcomes.
• Coordinate referrals to substance abuse, mental health services, and other social services.
