BayMark Health Services in Maryland is seeking a compassionate Substance Abuse Counselor to support patients through medically assisted programs and coping strategies for opioid addiction. Responsibilities include assessing eligibility, involving families in recovery, creating treatment plans, addressing grievances, and developing aftercare and discharge plans.

Qualifikationen

• Certification or license is required (ADt, CAC-AD, CSC-AD, LCPC, LCSW, LGADC or LCADC) from the Maryland Board.
• Satisfactory criminal background check and drug screen.

Aufgaben

• Conducts initial assessment and evaluation interviews for service eligibility determination.
• Involve families and significant others in patient’s recovery process whenever possible.
• Facilitates, establishes and reviews with each patient the initial treatment plan.
• Respond to patient grievances and complaints as needed.
• Develop aftercare plans and discharge plans.
• Other related duties as determined by supervisor.
