Job summary

Crossroads Treatment Centers is seeking a Substance Abuse Counselor at the Fountain City clinic in Knoxville, TN. The role focuses on individualized counseling, clinical assessments, treatment plan design, care coordination, and discharge planning to support client recovery.

The ideal candidate will have general experience serving underserved populations and may have lived addiction experience. Opportunities for training and advancement within Counselor levels are available.

Qualifications

• General experience working with underserved populations.
• Lived addiction experience is a plus.

Responsibilities

• Provide individual counseling to highly complex patients.
• Conduct clinical assessments to assess treatment needs.
• Design and update patient treatment plans including care coordination, referrals and discharge planning.
• Manage assigned caseload and ensure compliance with treatment requirements.
• Collaborate with nurses and providers to deliver holistic care.
• Facilitate urine drug screening for patients.
