Job summary

Outreach Development Corporation is seeking a Case Manager in Brentwood, NY to deliver care coordination services for individuals with histories of substance abuse. You will work within an interdisciplinary treatment team to coordinate services, provide crisis intervention, and advocate for clients to access necessary supports.

Qualifications

• 1 year of experience working with clients with substance abuse disorders.
• 1 year of experience working with adolescents with co-occurring and mental health problems.
• 1 year of experience working in an OASAS funded facility is a plus.
• CASAC-T/CASAC or higher preferred.

Responsibilities

• Case management and crisis intervention to coordinate treatment services.
• Facilitation of support groups such as life skills.
• Advocates on behalf of clients to obtain needed services.
• Interfacing with DSS, probation, courts, and other agencies.
