Job summary

Outreach Development Corp. in Bellport, NY seeks a Case Manager responsible for delivering care coordination services to clients with a history of substance abuse. You will be part of an interdisciplinary treatment team and provide direct case management and crisis intervention.

Lead efforts to connect clients with housing, transportation, and other supports, coordinate with agencies (DSS, probation, courts), and document progress in treatment planning. CASAC credentials preferred.

Qualifications

• CASAC or CASAC-T certification preferred.
• 1 year experience with clients with substance abuse disorders.
• 1 year experience with adolescents with co-occurring and mental health problems.

Responsibilities

• Provide case management and crisis intervention to coordinate treatment services.
• Facilitate life skills and group sessions.
• Advocate for clients to obtain needed services.
• Interface with DSS, probation, courts and other agencies.
• Maintain treatment planning and communication with service providers.
