Job summary

Anne Arundel County Public Schools seeks a Secondary Transition Facilitator to support students with disabilities as they move from school to postsecondary paths. You will lead IEP-driven transition planning, coordinate services, build community partnerships, and guide student-centered planning for students age 14 and older.

Qualifications

• Bachelor's Degree preferably in Special Education or related field.
• Three (3) years relevant experience with students with disabilities.
• Knowledge of IDEA, COMAR, and MSDE requirements.
• Strong collaboration and communication skills.
• Ability to coordinate transition services and agencies.

Responsibilities

• Lead the development and implementation of age-appropriate transition plans.
• Support IEP teams in selecting transition assessments and setting postsecondary goals.
• Establish community partnerships to expand transition opportunities.
• Coordinate work-based learning experiences and career exploration.
• Ensure compliance with IDEA, COMAR, MSDE and district procedures.
