Job summary

This is a full-time, day-shift role with responsibilities spanning referral screening, patient assessment, diagnostic coordination, and care plan documentation. The RN Navigator will coordinate across the patient journey for procedures like TAVR and MitraClip, collaborating with referring providers and participating in weekly conferences.

Qualifications

• Bachelor's Degree in Nursing required.
• California RN license required upon hire.
• National Provider BLS - AHA required upon hire.
• Minimum 3 years cardiovascular nursing experience.

Responsibilities

• Coordinate care across the patient journey for structural heart procedures (TAVR, MitraClip, LAAO, valve interventions).
• Handle referral screening, patient assessment, and diagnostic coordination.
• Schedule clinics, prepare charts, educate patients, and document care plans.
