Job summary

MarinHealth Nurse (RN) Navigator, Cardiology, Structural Heart coordinates care from referral and test scheduling to treatment options for TAVR, LAAC, TEER, and PFO/ASD closures. The role reports to the Clinical Program Director for Cardiology and ensures robust documentation and CMS/NCDR compliance.

Responsibilities include educating patients, coordinating with physicians, and guiding transitions of care from hospital to home. Requires BSN and California RN license, with a 40-hour workweek.

Qualifications

• Bachelor's Degree of Science in Nursing from an accredited college/university.
• Minimum two years cardiovascular clinic or hospital nursing experience.
• Two years critical care hospital experience preferred.
• Two years case management experience in the hospital or clinic setting preferred.
• CPR and ACLS certification.
• Current Registered Nurse California license.

Responsibilities

• Maintains referral and database using Microsoft Excel and TEAMS, and maintains compliance documentation for CMS and NCDR.
• Educates patients about their cardiovascular condition and explains treatment options as directed by the cardiologist.
• Triages patients’ symptoms/arrhythmias and consults with cardiologists per protocol.
• Supports Transition of Care Management to ensure discharge medications and instructions are understood and follows up with cardiologists.
• Maintains audit-ready documentation and promotes patient safety and adherence to clinic protocols.
