Job summary

Bon Secours Mercy Health in Virginia seeks an experienced RN Navigator for the Structural Heart program at St. Mary's and Memorial Regional.

The role guides patients through the healthcare system, helps with scheduling, and connects them with resources to ensure a smooth care journey. You will coordinate care across teams, support discharge planning, and participate in patient education and quality initiatives.

Qualifications

• Bachelor’s degree in nursing (preferred).
• Licensed Registered Nurse in state of practice (required).
• 3 years of direct patient care experience as an RN (required).
• BLS Basic Life Support – American Heart Association (preferred at hire).
• Oncology Nurse Navigator (ONN-CG) and Breast Health Navigator (preferred) if placed in those departments.

Responsibilities

• Guides patients in understanding diagnoses, treatment options and the healthcare process.
• Serves as the essential communication link between patients and care providers.
• Coordinates and advocates for care across multidisciplinary teams.
• Monitors patient progress and adjusts treatment plans as needed.
• Facilitates appointments and connects patients with support services.
• Participates in patient education programs and quality improvement initiatives.
