Virginia Heart in Falls Church, VA is seeking a Structural Heart Nurse Navigator to provide primary clinical support for structural heart patients in the clinic and coordinate with the valve program. Travel to affiliated offices may be required to ensure seamless patient care. The role requires an RN licensed in Virginia with 2 years of cardiac care experience preferred.

Qualifications

• Current Virginia State RN license in good standing.
• 2 years of clinical experience in managing cardiac patients preferred.
• BLS required; ACLS preferred.
• Computer literate with Excel; experience with EHR systems.

Responsibilities

• Supports structural heart provider in clinic with patient pre-screening and testing prep.
• Performs STS risk scores and gathers patient history and vitals.
• Coordinates scheduling of procedures and follow-up with hospital schedulers.
• Educates patients and families and supports nursing staff training.
• Acts as liaison between physicians, patients, and referral facilities to ensure continuity of care.
• Maintains documentation and ensures regulatory compliance for procedures.
