Job summary

Virginia Heart is seeking an experienced Structural Heart Nurse Navigator to provide primary clinical support for the Structural Heart team in our outpatient cardiology setting. The role includes patient-centered care, coordinating testing and follow-up, and collaborating with the structural heart provider.

The RN must have Virginia licensure, strong communication, and proficiency with EHR and Excel. Travel to other offices may be required and orientation is provided.

Qualifications

  • Successful completion of a Registered Nurse program.
  • Current Virginia State Registered Nurse License in good standing.
  • 2 years of clinical experience in the management of cardiac patients preferred.
  • BLS required; ACLS preferred.
  • Excellent written and verbal communication skills.
  • Proficient in Excel and computer literacy.
  • Experience with Electronic Health Record (EHR) is preferred.

Responsibilities

  • Assist the provider with patients in the office and perform pre-screening and testing results collection.
  • Perform STS risk scores and obtain chief complaint, vital signs, and ECG as indicated.
  • Review medications and allergies, update medical histories.
  • Coordinate scheduling, follow-up, and testing with structural heart procedures.
  • Serve as liaison among patients, families, and treating physicians.
  • Maintain documentation and ensure regulatory compliance.
  • Provide patient and family education and promote staff education.
  • Shadow the structural heart provider to stay current with processes.

The Structural Heart Nurse Navigator provides a deliberate patient and family centered partnership with the healthcare team and facilitates a seamless patient experience between the Inova Valve Clinic and Virginia Heart. The Structural Heart Nurse Navigator may be required to travel with the structural heart provider as needed.The Structural Heart Nurse Navigator may be required to cover other offices as needed.

RESPONSIBILITIES:
  • Assists the provider with patients in the office to include but is not limited to the following tasks:
  • Clinical Chart Preparation, focusing on pre-screening and obtaining testing results for structural heart consultations.
  • Performs STS risk scores on patients as indicated.
  • Obtains chief Complaint or reason for the visit.
  • Obtains Vital Signs
  • Performs an ECG as indicated.
  • Reviews current Medication List and allergies, update as needed
  • Updates Past Medical History and Social History
  • Updates Review of Systems as indicated.
  • Receives GEMMS messages from the structural heart provider with concise interpretation and follow up instructions. Contacts patients accordingly regarding results and follow up.
  • Follows and maintains a current spreadsheet of patients going through the structural heart consultation process and facilitates appropriate follow up and procedure scheduling based on the provider’s instruction.
  • Organizes and submits procedure data to ensure compliance with regulatory guidelines.
  • Participates in ongoing assessment of all structural heart patient’s pre and post op.
  • Provides and promotes patient and family education as well as promotes and encourages nursing staff education and involvement.
  • Serves as a liaison between treating and referring physicians, the valve clinic, patients, families, and referring facilities to promote quality and continuity of patient care.
  • Reviews patient plan of care with the interdisciplinary team and with the patient and family.
  • Assumes clinical management responsibility for the specific patient population in collaboration with the structural heart provider.
  • Ensures patients have completed all necessary pre-procedure testing and helps coordinate scheduling tests as needed.
  • Facilitates / coordinates scheduling of structural heart procedures with the hospital schedulers.
  • Facilitates/ coordinates scheduling of office visit and valve clinic appointments as needed.
  • Performs telephone pre-screening, triage, and answers inquiries for structural heart patients.
  • Shadows the structural heart physician as needed to stay current on new processes and procedures.
  • Has frequent communication with the provider schedulers regarding opening and closing the structural heart providers books as needed.
  • Works as a resuscitative team member in emergency situations, including administration of IV medications.
  • Upon successful completion of the Prescription Refill Authorization Competency Exam, authorizes prescription refills under the provider's supervision.
  • The structural heart RN orientation is one month. The orientation includes all the listed duties above. Orientation is a progressive process.
  • One on one training will be provided with the Structural Heart Provider
  • The RN is expected to attend training sessions of new Virginia Heart processes as they are implemented.
  • Continuing education is not mandatory but is encouraged and expected.
  • Opportunities for continuing education are ongoing and made available to clinical staff.
  • Reports general medical supply needs to the office manager. Maintains and cleans clinical equipment per manufacturer’s recommendations. Notifies office manager of equipment failures in a prompt manner.
  • Expected to read, understand, and follow Virginia Heart Employee Handbook, the Clinical Policy, and Procedure Manual, including updated policies when indicated.
  • Demonstrates professionalism in all interactions with patients, providers, and fellow employees to create a positive experience.
  • Adheres to the Virginia Heart dress code policy. Exudes confidence and positive body language.
  • Resolve issues appropriate to position. Maintains workstation and all clinical areas in a clean, tidy fashion.

REQUIREMENTS:
  • Successful completion of a Registered Nurse program
  • Current Virginia State Registered Nurse License in good standing
  • 2 years of clinical experience in the management of cardiac patients preferred.
  • Preferred experience or knowledge of general cardiology, valvular disease, and surgical and non-surgical treatment approaches.
  • Current Basic Life Support (BLS) required; Advanced Cardiovascular Life Support (ACLS) preferred.
  • Computer Literate, proficient in Excel.
  • Excellent written and verbal communication skills and organizational skills.
  • Strong ability to work independently and to multi-task in a fast-paced environment.
  • Prior experience with the use of an Electronic Health Record (EHR).
  • The structural heart RN must successfully pass the Prescription Refill Authorization Competency exam to authorize prescription refills.
