Inova Health is seeking a dedicated Registered Nurse (RN) Patient Care Navigator - Structural Heart for a full-time role in Fairfax East. Mon-Fri, no holidays or weekends. Relocation assistance eligible.

Qualifications

• Education: Bachelor's Degree in Nursing (minimum)
• Experience: 3 years nursing experience
• Certification: Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support from the American Heart Association

Responsibilities

• Coordinates a patient’s plan of care including appointment scheduling, attendance, transportation, referrals, and answering questions.
• Acts as liaison and advocate to help patients access services and achieve best health outcomes.
• Refers cases to appropriate resources and anticipates patient needs and rights.
• Conducts comprehensive assessment of health and psychosocial needs and develops a case management plan with patient and support system.
• Educates patient, family, and health-care team about treatment options, resources, and insurance benefits to support informed decisions.
• Collaborates with multidisciplinary teams to address questions about care and treatment options.
• Develops and maintains reliable systems to document and monitor patient data.
