Sentara Norfolk General Hospital is seeking a full-time, temporary Registered Nurse (RN) Stroke Program Coordinator/Navigator to drive patient progression and reduce length of stay for stroke patients in Norfolk, VA. This role leads daily operations, collaborates across disciplines, and has the potential to become permanent based on measurable impact.

The position requires a BSN, RN license, BLS within 90 days, and 3 years of RN experience, with a focus on coordinating care and ensuring timely,

Qualifications

• BSN required.
• Registered Nurse license required.
• Basic Life Support (BLS) required within 90 days of hire.
• 3 years of RN experience required.

Responsibilities

• Daily patient surveillance to support timely progression of care for stroke patients.
• Identify and escalate delays related to diagnostics, consults, discharge planning, or clinical changes.
• Recognize high-risk patients early and escalate concerns related to decline or complications.
• Coordinate care across neurology, hospitalists, rehab, case management, nursing, and other partners.
• Support early planning for transitions to inpatient rehab, SNF, home health, or other settings.
• Prompt early palliative consults for high-risk populations (SAH/ICH as appropriate).
• Assist with post-discharge continuity and advance care planning as needed.
