Job summary

Sentara Norfolk General Hospital is seeking a full-time, temporary Registered Nurse Stroke Program Coordinator/Navigator to drive patient progression and improve outcomes for stroke patients. This role partners across disciplines to ensure timely, safe transitions and to reduce length of stay during the eight-month assignment.

The incumbent will lead day-to-day operations, coordinate care with neurology, hospital medicine, rehab, case management, nursing, and palliative teams, and help determine

Qualifications

• BSN required; RN license; BLS within 90 days of hire.
• 3 years of RN experience required.
• Stroke knowledge and commitment to education are expected.

Responsibilities

• Daily patient surveillance to support timely progression of care.
• Identify and escalate barriers to diagnostics, consults, discharge planning.
• Mitigate mortality risk by early recognition of high-risk patients.
