Yale New Haven Health System is seeking a Physician Assistant or Nurse Practitioner with a strong interest in stroke and neurosciences. The role combines inpatient and outpatient care across the Adams Neuroscience Center, with a four 10-hour shift pattern and no overnight/on-call coverage.

New graduates are encouraged to apply along with experienced APPs in Stroke, Neurology, Neuro-ICU, or related high-acuity inpatient settings.

Formación

• Graduate from an accredited NP/PA program.
• Active license or eligibility to practice as NP or PA.
• Interest in stroke and neurosciences; new graduates welcome.

Responsabilidades

• Assess, triage, and manage patient care as part of an interdisciplinary team.
• Collaborate with physicians and other clinicians across inpatient/outpatient settings.
• Educate patients and families and participate in discharge planning.
• Provide continuity of care across stroke patient journey in inpatient and outpatient clinics.
• Support quality and safety initiatives within the neuroscience program.
