Job summary

Mental Health Association of Rochester is seeking a Stroke Program Coordinator to lead and coordinate services and activities for the stroke program.

You will act as a liaison among physicians, nursing staff, allied professionals, patients and families to support communication and coordinated care. Working with a multidisciplinary team, you will facilitate stroke patient care across the continuum and ensure clinical care pathways are followed.

Qualifications

• Experience coordinating clinical programs and care pathways.
• Ability to liaise with physicians, nursing staff and families.
• Strong communication and organizational skills.

Responsibilities

• Coordinate and implement program services and activities for stroke care.
• Serve as a liaison between clinical teams and patients/families.
