Job summary

Phoenix Rescue Mission in Mesa, AZ is seeking an outreach-oriented case manager to connect with homeless individuals, help them access services, and develop personalized plans. The role emphasizes a credible Christian witness and community engagement.

You will collaborate with stakeholders, document client information in the database, and participate in devotions and organizational activities while maintaining composure in challenging situations.

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, Counseling, or related field, or 4 years of relevant experience.
• Certified in Peer Support, preferred.
• Valid Arizona driver's license with 2+ years driving record (MVR check) and ability to qualify for company insurance policy.
• Knowledge of street homelessness and resources to exit homelessness.
• High level of composure and ability to remain calm in stressful situations.
• Discretion and confidentiality in handling client information.
• Excellent communication skills.
• Proficiency with Microsoft Office applications.
• Solid Christian witness and lifestyle aligned with the Mission.

Responsibilities

• Connect with and motivate people living on the streets to strive toward self-sufficiency.
• Assist homeless individuals in navigating the social service system to identify resources.
• Set goals and develop individualized case plans for outreach prospects.
• Establish and maintain relationships with community stakeholders and attend outreach events.
• Ensure client information is entered timely and accurately in the PRM database.
• Lead or participate in devotions, prayers, or other organization activities promoting a Christian witness.

Education

Bachelor's degree in Social Work, Sociology, Psychology, Counseling, or another social service field
