Job summary

University of Southern California's Keck School of Medicine Street Medicine Program seeks a Community Health Worker to help patients access community services, provide social support, and promote healthy behaviors. This full-time, on-site role in Los Angeles involves after-hours work and collaboration with the care team.

Responsibilities include outreach, follow-up, referrals, and coordinating appointments with providers.

Qualifications

• Minimum education: High school or equivalent.
• Experience: 1 year in a relevant role.
• Valid California driver’s license; knowledge of LA County areas and freeways.
• TB test and updated immunization records for LA County credentialing.

Responsibilities

• Drive team to rounds in the street medicine program and maintain the vehicle.
• Support street visits with safety awareness and patient engagement.
• Provide outreach, follow-up, and help patients attend appointments.
• Refer patients to community services and transportation resources (transportation is prohibited to transport patients).
• Assist with completing applications, enrollment, and eligibility for uninsured patients.
• Coordinate with the medical team to ensure comprehensive care and follow-up.
• Record care management information in EMR within 24 hours after contact.
• Attend meetings and manage an assigned caseload.
