Responsibilities

• Complete psychosocial assessments for residents upon admission and as needed.
• Develop and implement individualized care plans with the interdisciplinary team.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning, referrals, and coordination of community resources.
• Advocate for resident rights and ensure regulatory compliance.
• Facilitate family meetings and care conferences.
• Support adjustment to illness, disability, and long-term care placement.
• Maintain accurate documentation per facility policies and regulations.
• Assist with grievance resolution and participate in QAPI and facility improvement.
