Job summary

BoldAge PACE is seeking an Outreach Ambassador to drive program growth through strategic outreach, relationship development, and high-quality prospect engagement. This role targets identifying, qualifying, and converting prospective participants while educating the community about the PACE model of care.

Qualifications

• High school diploma required; Bachelor’s degree in Marketing, Communications, Business, Healthcare Administration, or Human Services preferred.
• Minimum of 3 years in community outreach, relationship-based sales, or business development — ideally in healthcare, aging services, or nonprofit settings.
• Proficiency in CRM systems and Microsoft Office Suite.
• Knowledge of PACE, Medicare/Medicaid eligibility, and local senior service systems preferred.
• Proficiency in communication and public speaking.

Responsibilities

• Develop and execute strategic outreach plans to meet growth goals.
• Identify, qualify, and cultivate high-quality referrals from partners.
• Conduct consultative conversations with prospects and caregivers.
• Maintain documentation of outreaches, referrals, and activities in the CRM.
• Represent BoldAge PACE at community events and meetings.
