Job summary

Masonic Home Union City is seeking a skilled MDS Coordinator to oversee MDS/RAI assessments, care planning, and ARD scheduling in a SNF. You will ensure regulatory compliance, maximize reimbursement, and collaborate with the IDT to support residents’ care needs.

Requirements include a CA RN license, MDS/RAI experience, and strong documentation abilities. Proficiency with CMS rules and Medicare PPS is essential for success.

Qualifications

RN with CA license and a background in long-term care.
Experience with MDS/RAI and PPS processes.
Strong documentation and communication skills in a regulated setting.
Proficiency with computer systems and MS Office.
Leadership/management experience in a healthcare team.

Responsibilities

Set ARDs for the MDS process with IDT involvement.
Lead PPS meetings to optimize reimbursement and care plans.
Coordinate with IDT for Medicare coverage and termination notifications.
Monitor CMS timelines and MDS submission compliance.
Prepare monthly care plan calendars and conduct care plan conferences.
Review incidents and ensure care plans reflect changing needs.
