Job summary

Community Care of North Carolina is seeking a Maternal Child Health Program Manager to oversee statewide care management for children 0-5 and high-risk pregnant CCN attributed members, ensuring fidelity to the Maternal & Child Care Management model and collaboration across PHPs, LHDs, and CCNC care management staff.

The role leads LHD and internal care management to ensure adherence to the Maternal & Child Health Policy Manual, integrating physical health, behavioral health, and social services

Qualifications

• Bachelor’s degree required.
• RN licensure required.
• CCM certification required.
• 3+ years of care management experience or 3+ years of management/supervision experience in maternal/pediatric populations.

Responsibilities

• Provide leadership in staff management including recruitment, training, evaluation and development.
• Develop and implement policies and procedures aligned with contractual requirements.
• Oversee care management activities across LHDs and CCNC teams to ensure quality and cost-effective care.
• Lead quality improvement initiatives and monitor performance metrics.
• Supervise Local Health Department Supervisors and coordinate cross-functional workgroups.
• Ensure privacy, security, and ethical data handling in all activities.
• Mentor staff and drive continuous professional growth.
