Qualifications

• Master’s degree in social work, mental health, nursing, or related field required.
• Minimum two years in healthcare, managed care, or related roles in patient assessment, treatment planning, UM, or case management.
• Strong collaboration with treatment teams and payers for appropriate levels of care.

Responsibilities

• Review clinical documentation to support continued stay authorizations.
• Assess treatment plans and advocate for additional services as indicated.
• Promote efficient use of resources for patients and uphold patient rights.
• Maintain contact with attending physician, program manager, nurse manager, and team.
• Collaborate on continued stay and discharge planning; ensure placement at appropriate level of care.
• Coordinate with program staff to facilitate smooth transfer or discharge.
