CHOICE of NY seeks a Senior Program Director to supervise the Health Home Care Management Program and the Criminal Justice Health Home component, ensuring high-quality, client-centered services.

The role leads cross-department collaboration to achieve measurable outcomes, represents the program at external meetings, and supports operational and financial performance. Hybrid schedule, compensation range $70,000–$80,000.

Qualifications

• Bachelor’s degree in social work, human services, psychology, sociology, or related field preferred.
• 3 years of experience in operating or supervising social services programs, preferably in care management.
• Strong familiarity with care coordination/care management; CMAs and Health Home preferred.
• Excellent interpersonal and written/verbal communication skills.
• Commitment to quality programs and excellence in organizational and project management.

Responsabilités

• Develop internal and external relationships that result in program growth and enrollment.
• Ensure administrative tasks, policies, and procedures are implemented and adhered to by staff.
• Review policies and procedures and conduct periodic reviews and updates to ensure compliance with Health Home and governmental regulations and best practices.
• Monitor staff documentation to ensure compliance with program policy and procedure.
• Supervise Health Home Supervisors and other program staff to ensure quality care management services.
• Oversee Cabrini Case Management and grant spending.
• Provide overall oversight and strategic direction of the Criminal Justice Department.
• Build long-term, cross-sector relationships to sustain or expand CHOICE of NY’s initiatives.
• Lead and mentor a high-performing team with ongoing professional development.
• Prepare narrative or quantitative data reports for funding sources or internal departments.
