Qualifications

  • 3 years of experience in operating or supervising social services programs, preferably in care management.
  • Strong familiarity with care coordination/care management; CMAs and Health Home preferred.
  • Excellent interpersonal and written/verbal communication skills.
  • Commitment to quality programs and excellence in organizational and project management.

  • Ensure administrative tasks, policies, and procedures are implemented and adhered to by staff.
  • Review policies and procedures and conduct periodic reviews and updates to ensure compliance with Health Home and governmental regulations and best practices.
  • Monitor staff documentation to ensure compliance with program policy and procedure.
  • Supervise Health Home Supervisors and other program staff to ensure quality care management services.
  • Oversee Cabrini Case Management and grant spending.
  • Provide overall oversight and strategic direction of the Criminal Justice Department.
  • Build long-term, cross-sector relationships to sustain or expand CHOICE of NY’s initiatives.
  • Lead and mentor a high-performing team with ongoing professional development.
  • Prepare narrative or quantitative data reports for funding sources or internal departments.

Job description

The Senior Program Director will provide supervision to the Health Home Care Management Program, including oversight of the Criminal Justice Health Home component. This role includes direct supervision of program staff to ensure high-quality, efficient, and client-centered care management services. The Senior Program Director will lead and collaborate across departments to ensure the delivery of measurable outcomes. Additionally, this position will represent the program at external meetings with the lead Health Home, governmental agencies, and community partners to support operational processes that promote optimal programmatic and financial performance.

Essential Functions:

  • Developing internal and external relationships that result in program growth and enrollment.
  • Ensuring administrative tasks, policies, and procedures are implemented and adhered to by program staff.
  • Review policies and procedures and conduct periodic reviews and updates as needed to ensure compliance with Health Home and governmental regulations and best practices.
  • Monitor reports on staff documentation, including consents, intakes, baseline screenings, functional assessments, plans of care, progress notes, and case closures, to ensure compliance with program policy and procedure.
  • Supervising Health Home Supervisors and other program staff to ensure the delivery of high-quality and efficient care management services.
  • Oversee Cabrini Case Management and grant spending.
  • Responsible for the overall oversight and strategic direction of the Criminal Justice Department.
  • Ensure ongoing programmatic excellence.
  • Build long-term, cross-sector relationships that help sustain or expand the impacts of CHOICE of NY’s initiatives.
  • Lead, mentor, and cultivate a high-performing team, fostering an environment of continual growth and professional development.
  • Completing monthly, annual, or other periodic requests for narrative or quantitative data reports for funding sources or internal departments.
  • Meeting with clients individually and facilitating high-risk case conferences as needed.
  • Identifying trends in staff gaps of knowledge and making recommendations to the department on how to best address.
  • Overseeing and leading the efficient administration of the billing process with close attention to accuracy, quality service, and adherence to regulatory policies.
  • Conduct monthly meetings with program directors to collate and assess operational challenges, collaborate on strategic problem-solving, and oversee the implementation of effective solutions across all programs.
  • Collaborate closely with the Director of Data Operations and the Compliance Department to continuously enhance monthly reports, ensuring ongoing improvement in accuracy, relevance, and compliance with regulatory standards.
  • Special projects and other duties as assigned.

Qualifications for this Role:

  • Bachelor’s degree in social work, human services, psychology, sociology, or any related concentration preferred.
  • 3 years of experience in operating or supervising social services programs, preferably in care management.
  • Strong familiarity with care coordination/care management. CMAs and Health Home preferred.
  • Ability to deal with sensitive issues with diplomacy and discretion.
  • Unwavering commitment to quality programs and excellence in organizational and project management to achieve strategic objectives.

Work Schedule:Monday – Friday ; 35 hours per week. Hybrid schedule.

Compensation range: $70,000.00 - $80,000.00
