Job summary

Responsibilities

• Respond to medical problems presented by patients including diagnosis, investigation, treatment and referral as appropriate in line with guidelines and pathways
• Prescribe for patients, regularly review medications and monitor repeat prescriptions
• Provide medical leadership and expert advice to the wider healthcare team, engaging with the Practice Wellbeing approach
• Deliver effective patient care with high levels of communication with patients
• Identify and utilise evidence-based best practice through benchmarking and clinical guidelines
• Undertake appropriate clinical audit to improve clinical outcomes
• Inspire and develop collaborative relationships across the multidisciplinary team, ensuring clinical governance is incorporated into day-to-day practice
• Promote multidisciplinary discussions around complex case management, ensuring systems for managing results, letters and tasks
• Undertake clinical risk assessments and implement management strategies for risks
• Share knowledge with the team through development sessions and governance meetings or case reviews
