Synergy HomeCare is seeking a Community Care Liaison to expand our reach in the community and build trust-based relationships with families, referral sources, and partners. You will lead live events, conduct presentations, and sign clients while maintaining a robust contact database to track activities and results.

Qualifications

• High School Diploma or GED required.
• 4+ years outside sales, event planning, or similar responsibilities.
• Proven success in building relationships that drive revenue.
• Excellent interpersonal, communication, and listening skills.
• Self-motivated with ability to work with minimal supervision.

Responsabilités

• Deliver live events and outreach activities (networking groups, health fairs, etc.).
• Cultivate relationships with referral sources and community partners.
• Maintain a contacts database and report on activities/results.
• Conduct presentations and meetings with prospects and referral sources to share information about Synergy HomeCare.
• Perform in-home assessments and sign clients.
• Support and grow our presence in three geographic areas.
