Job summary

Northwest Human Services (NWHS) is seeking a Community Partnerships Manager to build strategic relationships, collaborate with healthcare providers, public agencies, schools, and community organizations, and expand access to care in Marion and Polk counties. You will develop partnership strategies, coordinate cross-sector solutions, and monitor outcomes to improve services for underserved populations.

Qualifications

• Bachelor's degree in public health, healthcare administration, human services, social work, public administration, community development, communications, business, or related field.
• At least five years of progressively responsible experience in community partnerships, healthcare, human services, public affairs, coalition development, or cross‑sector program implementation.
• Experience translating strategic priorities into work plans, agreements, workflows, and measurable outcomes.
• Demonstrated success developing partnerships, facilitating complex discussions, resolving conflicts, and moving stakeholders toward action.
• Ability to interpret community assessments, demographic information, program data, and performance indicators.

Responsibilities

• Develop Partnership Strategies by creating and maintaining an annual plan aligned with NWHS priorities and community needs.
• Connect Partners and Coordinate Solutions by facilitating forums, listening sessions, and cross-sector meetings.
• Advance Community-Informed Care by identifying barriers and using community feedback to recommend service improvements.
• Strengthen Community Engagement and Advocacy by representing NWHS at events and monitoring policy developments.
• Measure Impact and Ensure Accountability by setting KPIs, tracking progress, and providing leadership with recommendations.
