Responsibilities

• Provide direct patient care to residents in post-acute rehabilitation and skilled nursing programs.
• Assist with activities of daily living (bathing, dressing, feeding).
• Monitor and record vital signs; report changes to the nursing team.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Maintain a safe and clean environment for patients and staff.
• Assist with medication administration, treatments, and procedures under supervision of licensed nurses.
