Job summary

Carastar Health is seeking a direct services professional in Alabama to screen clients for housing eligibility and deliver SUN assessments. The role includes coordinating social and housing services, transporting clients, and maintaining detailed client records.

The ideal candidate will have a behavioral science degree, knowledge of casework and confidentiality, and a willingness to work in non-traditional settings. A valid Alabama driver’s license is required.

Qualifications

• Bachelor's degree in behavioral science or related field.
• Some experience preferred.
• Must meet Medicaid Provider Status requirements.
• Must hold Alabama driver's license and maintain acceptable driving record.
• Must maintain liability coverage on personal vehicles.

Responsibilities

• Assist in screening individuals to determine housing eligibility.
• Visit referred persons and assess needs using SUN Assessment.
• Complete SUN Assessment Form based on client input.
• Identify resources from providers to meet identified needs.
• Prepare and review community mental health service plans quarterly.
• Coordinate social and maintenance services including daily living, transportation, habilitation, rehabilitation, prevocational and vocational, housing.
• Assist in obtaining income support services (housing, food stamps, SSI).
• Refer clients to additional services as needed.
• Transport clients to services and conduct outreach visits.
• Maintain 70% direct services productivity.
• Participate in required training to enhance professional skills.
• Provide mental health consultation to other agencies.
• Complete client records and required paperwork for Carastar.
• Inspect dwellings for cleanliness and rule compliance.
• Deliver case management per individualized service plans.
• Gather information for program certification/recertification.
• Put in work orders for repairs and maintenance.
• Perform other duties as assigned by supervisor.
