Job summary

Catholic Charities of Delaware, Otsego, & Schoharie Counties seeks a STEPH Case Manager to support individuals and families facing homelessness. You will assess needs, coordinate services, and monitor progress through regular contact and individualized planning.

Responsibilities include referrals, documentation, service planning, advocacy, and connection to housing resources. A compassionate, community-focused professional with a valid license is preferred.

Qualifications

• Associates Degree in Human Services or related field.
• 1–3 years of experience working with a diverse population.
• Valid driver’s license and access to reliable vehicle.

Responsibilities

• Accept and process referrals for individuals and families experiencing homelessness.
• Complete required documentation and verifications, including assessments and release forms.
• Develop personalized service plans with goals and referrals to local resources.
